Medication Information  


New York State Public Health Law requires a Health Care Provider's order for the the school nurse to administer over-the-counter or prescription medication(s) to a student. 

Medication Administration Information: 

  • Authorization of Administration of Medication form, completed by a Health Care provider and signed by a parent or guardian must be submitted to the child's Health Office.
  • All medication must be submitted to the Health Office by a parent/guardian or designated adult (over the age of 17)
  • School age children are prohibited from transporting medication on school transportation
  • All medication must be in a prescription bottle/vial clearly indicating the student's name, frequency, dosage and duration of the treatment. 
  • Medication that is ordered once, twice, or three times daily, may be managed from home
  • Parents or Guardians should report any changes in the course of the treatment, as well as any change in the child's health to the Health Office
  • In order for a student to take medication while on a school sponsored fieldtrip, the student must have a self medication form completed by the Health Care Provider and signed by the parent, as generally nurses do not attend field trips.
